What To Bring
Below is a list of information that is necessary to determine Food Stamp Eligibility. You should bring any information that pertains to you. The documents must not be more than four weeks old.
Things you need to provide, if they apply to you:
Personal identification such as:
- Photo identification Card, i.e. Drivers License, Sheriffs identification card, non-drivers state identification card with signature
- Medicare or Health insurance card
- Other current identification
- Alien Registration card
- Passport
Verification for all household members:
- Social Security number or having applied for one
- AND Birth verification
Residency and Shelter:
- Landlord form
- Lease agreement
- Property/school tax statement
- Statements from Salvation Army or any other shelter
- Statements concerning living and eating arrangements when you live with others
- Utility bills for heating, cooling, water, sewage, phone, electric or cooking gas
Income and deductions for all household members:
- Award letters or other acceptable proof of Social Security (SSA), Supplemental Security Income (SSI), New York State Disability, retirement or workman’s compensation
- Education income such as grants and loans (agency form completed)
- Last eight weeks pay stubs or statement from employer
- Verification of child support or alimony
- Unemployment verification (check stub or award letter)
- Statements from persons who contribute to your support
- Self-employment records, including the last federal income tax returns
- Verification of income received from, but not limited to, annuities, stock dividends, interest, lease or sale of property or other payments to household
Resources
- Title or registration for automobiles, boats, snowmobiles, motorcycles or any other registered vehicles
- Verified bank statement showing current account balance for all savings and checking accounts, Christmas club accounts, trust funds, Credit union accounts, certificates of deposits or any other accounts.
- Savings bonds
- IRA, Keogh accounts, or retirement accounts
- Stocks and bonds certificates
- Other documents that show household ownership or a resource
Employability:
- Medical statement that establishes any physical limitations that hinder employment
For adults under age 60 who cannot work due to medical reasons, the following should be submitted:
- proof that you receive Supplemental Security Income, Social Security Disability or disability-related Medicaid OR
- statement from medical professional which describes any physical or mental work-related limitations you may have
Medical Deductions:
- If you are sixty years or older, receiving Social Security Disability or Veterans
- Disability, bring in any bills for prescriptions, health insurance payments, doctors or other medical expenses
- SSI
